Crossroads Health is a 501(c)(3) nonprofit behavioral and primary health care provider serving the Northeast Ohio area with facilities located in Mentor, Painesville, and Willoughby, as well as our partner New Directions, an adolescent and young adult-based residential, recovery housing, and out-patient treatment facility for substance use and mental health disorders located at 30800 Chagrin Boulevard, Cleveland. Crossroads Health is an innovative leader for trauma-informed, mental health personalized care, primary health care, and recovery support. Crossroads Health employs 300 people to provide a continuum of integrated health services for all people, at all ages and stages of life, to better serve Northeast Ohio communities—no matter an individual’s ability to pay.
