Carmella Rose Health Foundation has been a pioneer and leader in community patient navigation since 2005, assisting individuals to find appropriate support during times of illness. A health crisis can significantly change the lives of individuals and their families. Although some people have the knowledge, skills, and support to manage, there are many others whose circumstances make them susceptible to falling through the cracks. Insufficient health insurance coverage, difficulty reading, lack of social support, fear, and poverty are but a few examples of barriers that can prevent people from accessing and receiving needed care. Delayed care or failure to receive proper care or treatments when needed may lead to poorer health outcomes. Even people with adequate resources can be under-served because of the complexities of the health care delivery system.
